It is a very difficult decision for me to write this letter, as it is time for me to share that I am currently on an extended leave of absence due to illness.

It has been the greatest privilege of my career to serve as your physician. Over the years, you have trusted me not just with your health, but with your stories, your milestones, and your lives. The deep connections we built during our appointments are what made my time in medicine truly special. 

My top priority is ensuring that your health remains in excellent hands. I am delighted to share that my colleague, Dr. Maziar Zamani, will be assisting me in this transition by being available to you for the next 30 days until 09/05/2026. He can be reached directly at 949-434-8412.  Dr. Zamani is a wonderful physician who shares my commitment to compassionate, thorough care. Of course, if you choose to transition your care to his practice, or to a doctor of your own choosing, we will gladly help transfer your files to your new provider.  I’ve included a Medical Record Release Authorization form in this letter so my office can properly release your records to your desired provider.  You may download it by clicking on this link:  Download Release Authorization Form
